All classes waiver


I/We, the undersigned, parent(s) and/or guardian(s) of the child attending this class (the Child), hereby consent to the participation of the Child, as a volunteer, in Speak Spanish in DC. If the Child should befall any injury entering, during, or leaving the Speak Spanish in DC class, I wish for the Speak Spanish in DC Instructor to take appropriate steps to immediately notify the emergency contact listed above. In the event that the emergency contact is unreachable, I authorize the Speak Spanish in DC Instructor to take any and all necessary steps to obtain medical treatment for my Child. I/We further discharge and release Speak Spanish in DC and its employees from any and all liability for injury, loss, damage, obligation, expense, or penalty sustained by the Child arising out of or in connection with the Child’s participation in the class.

Optional Photography Waiver


I/We hereby consent to and authorize the use and reproduction by Speak Spanish in DC of any and all photographs, recordings, videotapes, and/or other reproductions of the Child attending this class which have been secured by Speak Spanish in DC in order to document, exhibit, broadcast, and/or distribute or otherwise further reproduce said reproductions in whole or in part over or in any medium whatsoever, including, without implied limitation, newsletters, radio, newspapers, closed-circuit television, film, cable, and television, with or without compensation, in perpetuity.  I/We also release, discharge, and agree to hold harmless the producers or any persons, or entities acting under their permission or authority, from any liability arising from the use of these reproductions.

For Cooking, Science and Art Classes Only


I agree to allow some food preparation to be done in the home before class (if needed). I further agree that I have disclosed all of my child's food allergies on this registration form. I agree not to hold Speak Spanish in DC responsible if my child develops a food allergy during these classes.

Policies


*Refund policy: Refund requests must be received seven days before the beginning of classes. Any refund requests made will include a $25.00 processing fee. No refunds will be issued after that.
